From infancy through your child’s young adult years, the office of Dr. Alvin H. Prause will provide your child with quality health care, compassion, education, and support. Our team is committed to helping every patient achieve his/her optimum level of health and well-being. Through disease prevention, proper nutrition, and regular care, we are able to help parents raise healthy and well-adjusted children.

Appointment

Once you make an appointment at our office, that time is exclusively reserved for your child. If, for some reason, your child can no longer make their appointment, please call us with 24 hours notice so that we can reschedule the appointment.

Our office requires that a parent or legal guardian accompany the patient to the initial appointment. If you are the legal guardian, you must provide legal proof of guardianship. If documentation is not provided, we will not be able to honor the appointment time and it will be rescheduled.

What to Bring to Your Child's First Appointment

In order to provide prompt treatment to your child, please be sure to bring the following when you come in for their initial visit:

  • Photo Identification
  • Any medical records pertaining to your visit
  • Immunization Records
  • Insurance Information/Card

Please arrive 15 minutes before the scheduled appointment time to fill out necessary paperwork.

Insurance

We accept most insurance plans. Our office staff will file any necessary claims and work to get you the maximum amount of benefits allowed by your plan. If you have any questions about your insurance plan, feel free to call us so we can help you.

Payment

Payment is expected at the time services are rendered. For your convenience, we accept cash, checks, Visa, Discover, and MasterCard.
